Please help us celebrate National Healthcare Decisions Day, by completing your advance directive.

Our social workers will be at the Beloit Clinic (during the dates and times listed below) available to help you with any questions or concerns that you may have.  

Wednesday, April 19th | 9:00am – 3:30pm   &   Friday, April 21st | 9:00am – 3:30pm

We have spent 35 years caring for individuals at the end of their lives.  In that time frame we have witnessed many changes.  From advancements in medical technology to legal reforms.  One of the most beneficial, is of the later category.  With the passing of the Patient Self-Determination Act individuals were given the right to make decisions about future medical care.

We are encouraging you to take advantage of that right – now!  You are able to name an individual to make medical decisions on your behalf(should you become unable), the right to accept or refuse medical or surgical treatment and to plan and direct the types of health care you may receive if you become unable to express your wishes.

April 16 – 22 has been declared National Health Care Decisions Day Week (extended to a whole week, because that is how important it is). In honor of the holiday, we ask that you take the first steps in ensuring your loved ones are prepared and complete an Advanced Directive.
